 | Ring (08) 8396 2522 for appointment. Every effort will be made to accommodate your preference however emergencies will be given priority and our staff will attempt to contact you if there is a delay. We advise all patients to allow an hour in case of any delays caused by emergencies.
We believe that patients have the right to see their nominated doctor. While every effort will be made to accommodate patient's requests, there may be times, due to doctor absence, heavy demand or the nature of your health problem, that staff will offer you an alternative general practitioner. Doctors within the practice are able to access all your consult notes/investigations and can liaise with your regular doctors for your ongoing care.
Please advise the receptionist if a longer appointment is required for medicals, counselling, pap smears, problems of an extended nature or multiple problems. If you or a family member require an interpreter service, let us know when you book an appointment so that this can be organized.

After Hours Service |

 | The surgery number (08) 8396 2522 diverts, after surgery hours, to our deputising service. This is a 24 hour personalised service covered out deputising service after surgery hours. Routine after hours calls will be attended to by our deputising service, who report to the practice doctors. The deputizing service contacts surgery doctors 6:00am weekdays for urgent matters.

Cancelled Appointments |

 | Due to the increasing need for appointments, if you are unable to keep a scheduled appointment, it is important that you notify us as soon as possible. This will allow us to offer the appointment to other patients seeking medical assistance.
Failure to cancel an appointment, without reasonable notice, will incur a fee.

Fees |

 | It is desirable that the consultation fee be paid for on the day. However if that creates hardship, it is asked that the Gap Payment be paid on the day.
Eftpos, credit cards and cheques are accepted, and arrangements can be made to credit the rebate electronically to your nominated account. Please discuss with your doctor if you have any difficulty with these arrangements.
The current list of fees can be obtained from reception. Additional costs may be incurred where supplementary investigations, treatments, procedures or referrals are required, and the doctor will discuss these with you as required.

Home Visits |

 | If you have a condition that prevents you from attending the surgery and you require a home visit, please contact the surgery as soon as possible. This will enable staff to arrange an appropriate time for the doctor to attend.

Pathology Results |
 |
 | Patients are advised that it is their responsibility to telephone for all results. Please contact the surgery approximately 24 hours after you have your tests for doctor's comments, preferrably after 10am when the surgery is fully staffed.
Our reception staff are not medically trained and therefore will be unable to discuss the complexities of your results. If you require more information or further explanation, an appointment with your doctor is advised.
Please remember that under the Privacy Act, staff can only give results to the patient personally, unless previous permission has been given.

Prescriptions/Referrals/Sick Certificates |

 | For prescriptions, a consultation with a doctor is usually necessary as Government regulations required that regular assessments of patients are undertaken to ensure patient wellbeing. Exceptions must be approved by the doctor.
We also request that patients attend for repeat referrals unless a prior arrangement has been made with your doctor. Please ensure that appropriate notice is given (at least 5 days) prior to your specialist appointment.
Sickness certificates are a legal document and patients must attend for a consultation to obtain one. Certificates can not be issued retrospectively.

Reminders |

 | Our practice is committed to preventative care and we may send you a reminder notice at certain times to indicate that attendance for preventative health services is required. If you do not wish to be recalled please let reception know.

Telephone Access |
 |
 | Practice doctors will take emergency calls during consults. If your call is regarding a non urgent medical matter that requires discussion, the doctors would appreciate you making an appointment to see them. Other queries can be discussed with our receptionists who will endeavour to answer your query. If they are unable to answer your query, they will take your contact details and a practice staff member or doctor will contact you later with appropriate information. Due to consulting times, this may not be possible until the end of doctors sessions.

 | For Tea Tree Gully location - Disabled car parking is located at the rear of the building with access via ramps to the centre also located at the rear of the building. Please let staff know if you require assistance when making your appointment.
A wheelchair is located in the back waiting area to assist patients.
For Smart Rd Location - disabled car parking is located at the front of the building. Please let staff know if you require the wheelchair when making your appointment.

 | The practice has a privacy policy for the management of your health information which complies with the Privacy Act. Please feel free to request a copy of this policy from reception.

 | All instruments and needles are either disposable or thoroughly sterilized by accredited autoclaving.

Research / Educational Programs |

 | This practice participates in medical student training. The receptionist will, if possible, advise you that a third person will be present at your consult when booking your appointment, then at attendance at reception and again on entering consult rooms.
Please advise us if you are not comfortable with the presence of a third party.
Occasionally the practice participates in clinical audits using de identified information. If you have concerns about this, please talk to a staff member.
